- Practices must have fewer than four locations and must not be excluded from participation in any federal health care programs.

- The program runs from May 31, 2024 to December 31, 2024.
- Points are earned for Qualifying Jobs completed between April 9, 2024 and December 31, 2024.
- All points earned must be redeemed by March 31, 2025.

- For every pair of Transitions lenses sold, practices earn 5 points, with each point valued at $1.00.
- Points can be redeemed for a range of rewards, including a free pair of lenses, merchandising

Qualifying Sales:
- A Qualifying Job is a single sale transaction of Transitions lenses for a single patient, under one prescription, completed between April 9, 2024, and December 31, 2024.
- Sales that are paid for, in whole or in part, by federal health care programs, free product samples, remakes, or warranty fulfillments are excluded from earning points.
